For all in-office clinic consultations, we require referrals sent with a recent clinic note regarding the reason for the consultation and demographics sent to us before we can schedule an appointment for your patient.
You can also order a sleep study WITHOUT a consultation. If you just want the sleep study, please fill out one of the forms below, but please also send a recent clinic note and demographics with the order form. Our office will take care of getting insurance authorization for the test. Our sleep manager will contact the patient to schedule the study once insurance approval is obtained. If you are not certain what test to order, then simply check off "consult first" and we will take care of the rest!

Home Sleep Study Order Form
Please fill and send us this form to order a home sleep test for the evaluation of sleep apnea in any patient age 18 years or greater. If you desire a more in-depth sleep assessment, then consider an in-lab sleep study or sending the patient for a consultation first.
In-Lab Sleep Study Order Form
Please fill and send us this form to order in-lab sleep testing. You can choose to order the test only or a consultation if testing is abnormal. Please check off the desired test type. If you are not certain what test to order, then simply check off "consult first" and we will take care of the rest!
