Office Policy

  • New patient forms are sent by text AND email. They must be completed before your appointment.
  • All patients should arrive 10 minutes before the scheduled appointment time.
  • Bring your insurance card(s) and a method of payment for your visit.
  • It is YOUR responsibility to verify that our practice is in network with your medical insurance.
  • Co-pays, deductibles, and co-insurance are due at the time of service.  We do not accept personal checks.
  •  We will try to make accommodations should you arrive 10 minutes after your scheduled appointment time. If there is not enough time to provide proper care during the visit, we reserve the right to re-schedule your appointment.
  • Please understand that we have many patients trying to get an appointment so if you do not show up or provide less than 24 hours notice,  you will incur a $75 no show fee.  
  • Repeated failure to arrive for your appointment (3 consecutive cancellations and/or no shows) will result in discharge from the practice.
  • New patients that have missed 2 consecutive appointments will not be able to schedule another appointment at the practice.
  •  KEEP TRACK of how much medication and refills you have remaining. Allow up to 3 business days for refill requests to be completed.  Refills are not filled outside of office hours.  If you have a controlled prescription, you will need a follow up every 3 months to receive refills.
  • Completion of other paperwork, such as school forms, are subject a $15 fee and may take up to 7-10 days to complete.  
  • Results for EEGs and sleep studies may take 4-6 weeks. 
  • Dr. Quraishi is on call 24 hours a day, 7 days a week. Therefore, we ask that you call after hours only if your matter is urgent and you need to speak to the doctor.  There is no answering service.
