Patient Portal

We are pleased to offer our registered patients an online HIPPA compliant patient portal called “Elation Passport”.
The portal will allow 24/7 access to: 

• View personal health information, such as listed medications and allergies, summaries from past visits, and test results
• Look up your next appointment date and time
• Send and receive secure (non-urgent) messages to your doctor or office staff
• Request NON URGENT prescription refills
• Send and receive medical records via secure messaging

   

  IMPORTANT:
  For any urgent matters, please call the office. You must allow at least 3 business days for prescription requests to be completed and and at least 1 full business day for a response to any messages on the portal. If responses are not made in this time line, we request you call the office directly.
