Office Policies
Please take a moment to review our office policies. These guidelines have been implemented to enhance our efficiency and ensure that we can provide the best possible care to you. We appreciate your understanding and cooperation as we strive to improve our services.
- For every visit, please bring your insurance card and a method of payment. Co-pays, deductibles, and co-insurance are due at the time of service. We do not accept personal checks.
- It is YOUR responsibility to verify that our practice is in network with your medical insurance.
- Update us with any insurance, address, or phone number changes.
- Please arrive 10 minutes prior to your appointment time to allow you to be checked in to help ensure that most of your appointment time is spent with your clinician.
- If need to cancel your appointment, please do so before a 24-hour business day in order to avoid the cancellation fee.
- 3 no-shows or repeated cancellations in under 24 hours may result in being discharged from the practice.
- Prescription request should be on a weekday during business hours. Allow at least 3 business days to refill medications to allow time for the pharmacy to fill the prescription or work on a prior authorization if needed.
- When you call our office, you will have to leave a message, but please be assured that your call will be addressed within 24 hours. You may also leave a message on our patient portal for non-urgent messages.
- If you arrive 10 minutes past your appointment time, we may need to reschedule your appointment.
For New Patients:
a) view personal health information, such as listed medications and allergies, summaries from past visits, and test results
b) Look up your next appointment date and time
c) Send and receive secure (non-urgent) messages to your doctor or office staff
d) Request NON URGENT prescription refills
e) Send and receive medical records via secure messaging
